Florida voters form for changing party affiliation?

I just got the form in the mail that lets me make changes if things are wrong such as address or whatever. It says I can use the form to change my party as well. I wish to change my party affiliation, does anyone know if I have to fill out the form completely again just to change my party? I just want to make sure I'm not making any mistakes by filling everything out again. Thanks xD
